The goal of the Department of Social Services here at Bibb Medical Center is to provide comprehensive biopsychosocial supportive services.

Performing biopsychosocial assessments to determine physical, mental, emotional, and spiritual needs

Providing psychosocial supportive counseling and crisis intervention to residents coping with life-challenging medical diagnoses, chronic illness, situational anxiety/depression, and grief related to loss of ability

Coordinating post-acute care and comprehensive discharge planning in accordance to the needs of each individual and their family/caregiver by arranging home health, home hospice, and for the provision of home medical equipment

Assistance with health care decision-making and completing advanced directives/living wills

Referrals & education on community resources for financial assistance, supplemental insurance, medication assistance, housing options, nursing home placement, and special programs for older adults

Coordinating family meetings and care plan conferences with the interdisciplinary care team
